1 Where are we?

2 What area do you cover?

Our area for patients is largely confined to Coedpoeth, Bwlchgwyn and Gwynfryn. Please ask at reception for more details.

3 Who has access to your information?

Your physical records are retained within the surgery. Although technically belonging to the Minister of Health, these are not let out of the surgery except with your express permission or when you move from the surgery. Your electronic records are also retained on the premises, although a secure disk is sent every month to ensure that reliable backups are made of the information. Backups are kept on site but monthly backups are retained in secure locations off site. The doctors will usually ask your permission to discuss your case or notes with medical or nursing staff outside of the surgery. However, we rely on professional secrecy to prevent further dissemination of your information. You may refuse this discussion should you wish and we will abide by your request. The Government do not have access to your specific information unless there is a suspicion of fraud on your part or ours.

4 Do you work for the NHS?

We are independently contracted to the NHS but do not work directly for it. Our contract is with Betsi Cadwaladr University Health Board which can be contacted on 01978 346500.

5 How do we register as patients?

If you reside within our practice area (see above) we would ask that you attend the surgery and complete a registration form. You will normally be asked to attend a clinic with the nurse during which your medical details will be summarized and you medications recorded. You may also be given a quick examination of blood pressure, urine and whatever else the nurse deems appropriate on the day. It is a sign of your good faith to attend these booking clinics.

6 Do I have to stick to the same doctor?

No, you may elect to see anyone in the open morning surgeries or anyone with a free appointment in the later surgeries. As a general rule, it is probably sensible to see the same doctor for the same problem rather than chop and change, but that is up to you.

7 Do you have disabled access?

We do, but the surgery is cramped with sharp corners to narrow corridors. We have ramps and a disabled toilet and have recently bought an induction coil.

8 Who may be seen?

We will see anyone who turns up before the end of morning surgery or who makes an appointment for a later surgery. However, our contract (in the happy ignorance of common sense) states that we must record the following on this website - 'Registered patients aged 16 -74 who have not been seen for three years may request a consultation' and 'Registered patients aged over 75 years who have not been seen in the previous 12 months may request a consultation. If you are unable to attend the surgery for this consultation because of your medical condition, a home visit may be arranged'. Any such visits are at our discretion but we would never turn down a genuine request for a home visit to someone unable to attend because of ill health.

9 Data Matching Exercise?

The Welsh Audit Office currently (2009) requires each of the Local Health Boards in Wales to participate in Data Matching Exercises to assist in the identification of duplicate patient registrations within GP practices. The Local Health Boards have a statutory duty to provide the relevant information under the Government of Wales Act 2006. Each Data Controller (that's us) is required under the Data Protection Act 1998 to provide Fair Patient Notification ('so far as is practicable') to advise Data Subjects (that's you!) of this process.
